Using our referral form, you can let us know the name of the patient you referred and your own contact information. You’ll find a referral form below; simply fill it out with the required information, and we’ll send you a $20 Tim Hortons gift card once the person you referred has completed their appointment.

Disclaimer
Our Referral Program is only available to those who are currently patients at Fresh Dental. This applies to those who have completed an appointment at our clinic within the last 3 years.
To qualify for the Referral Program, the patient who you are referring to Fresh Dental must be a new patient (someone who has not made an appointment with us ever before). Only one referral credit will be awarded for each new patient. To receive your reward, the patient must schedule and attend their appointment and confirm to us that they were referred by the person claiming the referral credit. We will send the reward card in the mail once this has occurred. Gift cards are subject to availability.
We reserve the right to change or discontinue our Referral Program at any time without notice.
At our sole discretion, we may set a maximum amount of gift cards per referring person
